Nexus Primary Health has recently been funded through the Eastern Melbourne Primary Health Network for innovative after-hours and diabetes diversion projects. Both of these projects are about changing service delivery to better meet the needs of our communities.
We have a variety of roles available including this position of Health Systems Navigator which is part of the After Hours Project.

With a 40 year history, we are the "go to" organisation for primary health information and person-centred services in the outer and rural north of Melbourne.
Let's talk about the role
The Health Systems Navigator performs a vital role in establishing a medical neighbourhood approach in the local Wallan area. The role will provide a welcoming point of connection for "vulnerable populations" accessing our services out of usual hours.
Here is an opportunity for you to design responsive engagement, education and service options to support people in their access to health and community services and reduce their need to present to hospital emergency departments.
Let's talk about you
You have experience in developing community development approaches and have solid organisational skills including attention to detail and multi-tasking. You have a collaborative style of communication, working across all levels of health professionals. You'll draw upon your tertiary qualifications in either Community Development, Social Work, Welfare Work, Science or Health Sciences.
