Position Summary
The Community Connect Facilitator will work as part of the Kurrajong First Contact team and will:
• develop partnerships with Education sector to enable the smooth transition of people into support programs provided by Kurrajong across the Riverina and Murray regions including Griffith, Narrandera, Leeton and West Wyalong and surrounds
• leverage partnerships with Education sector to promote careers options for Support Workers across the Riverina and Murray regions

Position Key Responsibilities
It is expected that the Schools Connect Facilitator will perform the following;
1. Build and maintain strong linkages between Kurrajong and all local senior schools including State, Independent and Catholic education systems.
2. Assist the schools to have a clear understanding of the services provided by Kurrajong and the available pathways.
3. Delivery of the Schools Connect program to high schools across the region
4. To identify suitable school leavers that may be eligible for Services provided by Kurrajong, and identify suitable school leavers that may be eligible for a career at Kurrajong as a Support Worker
5. Organise tours of Kurrajong services with schools and prospective clients
6. Organise and present Career Days at schools willing to participate
7. Provide initial recruitment, orientation and disability awareness training to school peers as part of the peer support training package
8. Match volunteer peers with students with disabilities who share similar interests and reflect their aspirations and choices within their person centred activity plan
9. Assist people
10. Upskilling teachers and families about the NDIS and assisting with pension applications or NDIS access where required
11. Regularly evaluate the effectiveness of the Connect program by gaining feedback from its participants
12. Be responsible for Kurrajong’s professional presence at relevant expos across Griffith, Narrandera, Leeton and West Wyalong
13. Be responsible for all Admin process for Schools Connect across the regions
14. Other duties as required by the Kurrajong First Contact Manager, First Contact Coordinator or Community Connect and Employment Prep Coordinator.

Qualification, Certificates and Licences
1. Relevant qualifications for the position
2. Current Class C drivers licence
3. Working with Children's Check clearance
4. Willingness to undergo a National Criminal History consent check
Experience, Skills & Knowledge
1. Relevant qualifications and experience for the role
2. Experience working with young people and their families
3. Ability to build relationships with schools and partners in the community.
4. Excellent communication skills both written and verbal
5. Presentation and facilitation skills
6. Ability to think creatively and use initiative to work through issues 
7. Strong time management skills
8. Sound computer skills
