Care Coordination Centre, Dunedin Hospital

  • Permanent, full-time position

Our Single Point of Entry, Care Coordination Centre is passionate about the community it serves and strives to work effectively, whilst keeping our clients’ best interests at the top of our minds.  You’ll be joining a small, hardworking team and will play an important part in its success.

Your role will involve processing district wide referrals with a team of health professional colleagues skilled in assessing and triaging health information.  You will review and process referrals for entry criteria, clinical priority and ongoing allocation to other health professionals and their services or to health providers directly.  You will enjoy the challenge of a dynamic and unpredictable workday responding to ebbs and flows in referral demand.  As a Clinical Coordinator you will be managing a range of non-complex to complex health scenarios from requests for community nursing, allied health and health of older people assessments to palliative care funding.  You will work Monday to Friday and be available to respond to communications from GP’s and Hospital clinicians alike.

We’re looking for someone who is self-motivated, confident and proactive. It’s essential you’re a Registered Nurse or Allied Health Professional who holds current registration.  Our ideal candidate will have proven experience and demonstrate a high level of competence in verbal communication, assessment and facilitation of services for people who require community based treatment and support services.  You will have excellent problem solving skills and a ‘can do’ attitude, showing an ability to consistently apply access and entry criteria without sacrificing innovation or creativity.  It’s important you’re confident with computers and have experience with data entry, Outlook and other computer based systems.
