Healthvision is a leading provider of home-based healthcare solutions to enable people to return to, or continue living in, their own home by providing care and support for up to 24 hours a day, 7 days a week.
This role is part of a committed multi-disciplinary team that provides oversight and coordination to teams of Support Worker staff who deliver services to our clients. These range from home and personal care support to full nursing care for complex long-term clients.
Based in Hamilton, core working hours are Monday to Friday 8.30am to 5.00pm plus a share of an “on-call” roster for after-hours calls.
Tasks required:
- Development of care plans (with clinical supervision as required)
- Matching and rostering of support workers with clients
- Coordination of allocated support workers e.g. performance feedback, education and training
- Troubleshooting issues as they arise
- Involvement in the recruitment and retention of support workers
- Health and Safety assessments of client homes
- Relationship-building with clients and promotion of Healthvision
The work is challenging at times, varied and hugely rewarding as you are helping to make positive changes in people’s lives.
There is a strong emphasis placed on building relationships with clients and their support workers to ensure positive outcomes. Outstanding customer service skills, being empathetic and a good listener is essential to this role in addition to being able to negotiate competing demands.
This role will suit you if you are self-motivated and a fast learner, can remain calm under pressure, have an eye for detail, and approach everything with common sense.
To be successful in this role you will need the following:
- A health background, preferably with some knowledge of working with spinal injuries
- Proven scheduling and coordination experience
- Solid keyboard, database and Outlook skills
- Sound verbal and written communication skills
- Ability to engage, build and maintain productive professional relationships
- A ‘can do’ attitude and motivation to deliver outstanding service
- Highly organised with strong time management skills
- Team player attitude with the ability to work autonomously
- Full Drivers licence
